Effective July 1, 2021, schools will no longer issue work permits for minor employees (under age 18). Instead, employers who employ five or more minor employees (per location) will now be responsible for tracking and reporting minor-employee information via the state’s Youth Employment System (YES). Failure to comply with the new requirement may result in penalties up to $400 per infraction.
CHS Process to obtain a work permit (Until June 30, 2021)

To obtain a work permit the student must first secure employment. The student's employer must provide them with an Intention to employ form. The employer should have copies of this form already. If not, they can download this from the DOL website. The Employer must fill it out completely and sign it, the student signs it and the parent signs it. The student also needs to write on the form wherever they can because there is no spot for it; their DOB, POB and then print their parents name so that it is legible. The information from the intent to employ form then has to be uploaded into the DOL website and from there the work permit is generated. The work permit is then printed out and must be stamped with the school's official school stamp. It then needs to be signed by an administrator and signed by the student.
