Physical Night

Pre-participation athletic physicals will be conducted by the staff of Community Health Network in the Welch Activity Center on Wednesday, May 7, 2025. This is a multi-station screening for all Cathedral High School athletes (incoming as well) and focuses on injury prevention through pre-season examination. This physical will be good for all sports for the entire 2024-2025 school year.

The physical costs $25.00 and payment can be made by cash, check (made to Cathedral) or online at: gocathedral.com/athletics/physical-night. 

Parents and students must complete and sign all forms in Final Forms before taking the physical. Print out the official IHSAA PPE physical form from your Final Forms account and bring the physical form to be completed.

The schedule for physicals, by class, is:

• 6:00 PM: Current Freshman (2028) & Incoming Freshman (2029)
• 6:45 PM: Current Juniors (2026) & Current Sophomores (2027)

All Cathedral student athletes must have all documents in Final Forms completed/signed and a current IHSAA physical and on file in the athletic office prior to participating in any pre-season practice, conditioning, open field, or open gym activity.
