Final Forms

All student-athletes and their parents must be pre-registered with Final Forms, and have a current (dated after 4/1/2025) IHSAA physical on file before participating in any preseason workouts/open gym//practice.

1. Parents can register in Final Forms after April 1st, 2025 for the 2025-2026 school year. Go to: https://cathedralindianapolis-in.finalforms.com. And log in to your account.
2. Please be sure to register for the 2025-2026 school year 
3. Add the sport(s) your student plans on participating in during the 2025-2026 school year.
4. Complete/Update each form and electronically sign your name in the parent signature field. Click SUBMIT FORM after signing each page. You will see a FORMS FINISHED message when all forms are completed.
5. Your student will receive an email prompting them to sign required forms. They must electronically sign all forms.
6. Click on the tab titled “IHSAA PPE Physical (for physicians), then click on “Click to Print IHSAA Physical.” Take the printed form to an appointment with an MD/DO/NP/PA.
7. Forms that are completed at physical night will be kept on file in the Cathedral Athletic Office. Forms completed at an appointment with an MD/DO/NP/PA can be sent to the athletic office by uploading  in Final Forms, scanning/emailing to Cathedral Athletic Trainer, Mike Hunker at mhunker@gocathedral.com., or handing a copy of the forms directly to the Cathedral Athletic Office.
